Accident Report Form

Following an accident of any description involving club members or persons using club facilities an accident form must be filled out.

The accident forms can be found on the Maidenhead Hockey Club website, behind the bar at Maidenhead and Bray Sports Club in the accident folder and are also held by all club coaches.

Under the Data Protection Act 1998 once the forms have been filled out with personal injury details they must be keep secure. Therefore must be handed to either Men’s/Ladies/Colts Club captains. A photocopy can be made by the person that has incurred the injury for his or her records.

The form will be kept for a minimum of 3 years.
